Building an International Framework to Improve Reirradiation Care

Safe and effective reirradiation depends on accurate cumulative dose assessment and consistent reporting of prior treatments. Current practices for dose accumulation vary widely across Canadian institutions, leading to inconsistencies in cumulative organ-at-risk (OAR) dose estimations and, ultimately, uncertainty in clinical decision-making. As part of the COMP Reirradiation Initiative, a national working group is developing a framework to harmonize reirradiation practices across Canada. Over the next several months, this effort aims to produce expert consensus guidelines for standardized reporting, data sharing, and workflow processes to support safe and efficient reirradiation planning. This symposium will present the foundation and progress of this national effort. Drawing on results from the Canadian Patterns of Practice for Reirradiation – Case Study and Workflow Survey (2025) and lessons learned from recently published international guidelines, the session will explore strategies for achieving national alignment. Speakers will highlight opportunities to align with ongoing national efforts to standardize radiotherapy data, workflows and quality processes – laying out the foundation for structured data sharing, collaborative research and future clinical trials in reirradiation.
