Purpose
This study aims to assess the role of immobilization device design and material quality in improving radiotherapy precision for patients of all ages and treatment intents—from pediatric to adult cases and from curative to palliative care. The evaluation centers on several critical areas: setup accuracy, IGRT alignment, dose consistency, radiation exposure, and treatment workflowMethods: A structured assessment was conducted on widely used immobilization systems, examining aspects such as structural stability, repositioning reliability, compatibility with IGRT/CBCT technologies, dosimetric influence on PTV/CTV coverage, and how well the systems integrate into clinical workflows. For pediatric patients, additional metrics included tissue radiosensitivity, frequency of imaging, and long-term cancer risk. For adults and palliative care settings, the analysis focused more on anatomical variability, patient comfort, and speed of setupResults: Devices featuring greater structural rigidity, better anatomical fit, thermal resilience, and precise indexing significantly reduced both translational and rotational misalignments, limiting the need for couch adjustments and improving the accuracy of dose delivery. Overall treatment workflows benefited from quicker setups, fewer imaging repetitions, and fewer interruptions. Pediatric patients showed marked improvements in minimizing exposure to sensitive organs and reducing cumulative radiation from repeated imaging.
