Feasibility of Utps for Left-Sided Breast Cancer VMAT Planning on Dual-Layer Mlc Linear Accelerators

Abstract
Purpose

This study assessed the feasibility and dosimetric quality of uTPS (Shanghai United Imaging Healthcare, version R001) for left-sided breast cancer VMAT planning on dual-layer MLC Accelerators

Methods

Twenty patients were retrospectively analyzed: 10 post-breast-conserving surgery (BCS, 42.56Gy/16f + 9.6Gy/4f boost) and 10 post-modified radical mastectomy (MRM, 50Gy/25f). VMAT plans were generated using both Eclipse and uTPS optimized to meet identical clinical goals.Evaluated metrics included Left/Total Lung V5/V20Gy, Right Breast mean dose, Heart mean dose, and Spinal Cord Dmax. Left Humeral Head V20Gy was additionally evaluated for MRM. Plan complexity was assessed via Leaf Sequence Variability (LSV), Aperture Area Variability (AAV), and Modulation Complexity Score (MCS). Delivery accuracy was verified via ArcCHECK (3%/2mm). Data were analyzed using Wilcoxon signed-rank tests (p97%.

Conclusion

uTPS supports VMAT optimization for left-sided breast cancer on Halcyon. It provides plan quality comparable to Eclipse with improved sparing of specific OARs. Results validate the clinical feasibility of uTPS for dual-layer MLC Accelerators.
